TACHP aims to help families determine if their child’s current medical provisions are suitable for the child and fit within the bounds of a medical home. When appropriate, TACHP works with a family to identify the particular needs and desires of the child and family, including environmental contexts. TACHP strategizes with families to accessing a facility that operates medical home strategies or an established medical home. Staff work alongside individual families and service providers to address the identified barriers, including finding medically-appropriate transportation. TACHP encourages and support parents in becoming an active part in the medical home team by improving communication between the team and the family. TACHP reinforces an appropriate system for individual families, including organizing appointment dates, understanding scheduling policies, and identifying contact persons.
